Use of hemoadsorption therapy in neonatal surgery: A case of extreme Tetralogy of Fallot and severe pulmonary stenosis undergoing corrective surgery.

Evidence in the use of hemoadsorption therapies in adult and pediatric patients incorporated into the CPB circuit has shown that it is
associated with an attenuation of the inflammatory response. As a consequence, there is a reduction in the need for blood products and
vasoactive drugs. Until now, no cases have been reported in which this system is used in the neonatal population.
In this report we present the resolution of a neonatal case of 20 days old and 3.1 kg of weight, carrier of an extreme Tetralogy of Fallot with
severe pulmonary stenosis, which underwent corrective surgery, in which a hemoadsorption system in the cardiopulmonary bypass circuit was
used. Additionally, modified ultrafiltration post-extracorporeal circulation was also performed with the adsorber integrated into the circuit.
The evolution of the patient was satisfactory, being discharged from intensive care unit 48 hours after surgery and later home.
Although it is necessary to expand the evidence on the use of hemoadsorption therapy in neonatal cardiac surgery, the results in presented
in this case open’s a new horizon for its use, reducing the surgical impact in this population.
