Utilidad de la ecocardiografía speckle tracking para la evaluación de viabilidad miocárdica.

ABSTRACT
Objective: Myocardial viability is the best determinant of recovery of left ventricular function after revascularization. The objective of this study is to assess the usefulness of speckle-tracking echocardiography for the diagnosis of myocardial viability in patients with a diagnosis of myocardial infarction.
Method: An observational, descriptive, cross-sectional study was carried out in 23 patients with a diagnosis of myocardial infarction treated in the nuclear department of Cardiology and Cardiovascular Surgery Institute in Havana. Epidemiological, electrocardiographic, and echocardiographic variables and the correlation between single photon emission tomography and segmental longitudinal strain were analyzed for the myocardial viability study. The Kappa coefficient and the test validity measures were calculated.
Results: Mean age 58.7 years, male sex 82.6%, the most prevalent risk factors were hypertension (78.3%) and smoking (43.5%). The most frequent myocardial infarction location was the inferior wall (69.6%). A cut-off value of -12% for segmental longitudinal strain obtained a correlation coefficient kappa = 0.592, a sensitivity and specificity of 97.1% and 56.3% for the detection of myocardial viability compared to single photon emission tomography.
Conclusion: Speckle-tracking echocardiography proved to be a useful tool to assess myocardial viability.
