Abstract

Cardiovascular diseases are complications present in 0.2 to 4% of pregnancies and increasing, according to Directive of the European Society of Cardiology 2011. When clinical treatment is not enough, cardiovascular operation is recommended. The use of cardiopulmonary bypass increased the incidence of obstetric and fetal risks, and the in-depth knowledge of the surgical team about its repercussions allows the improvement of the surgical process to reduce these risks. With the aim of discribing, the management of cardiopulmonary bypass in a 30-week primigravida patient submitted to the exchange of mitral bioprosthesis for severe dysfunction. A case of 17-year-old woman is reported, primigravida in the 30th week who underwent surgical correction due to the presence of mitral bioprosthesis dysfunction with severe calcification. Initially, there was a schedule to accelerate fetal maturation, however, due to the clinical worsening, underwent an emergency operation with cardiopulmonary bypass, in which they took measures to reduce maternal and fetal risks. In conclusion, the cardiovascular emergency surgery with cardiopumonary bypass in pregnant women may occur provided that standardized protocols are followed and with the participation of a multidisciplinary team.
